This form is used specifically to evaluate prospective Vendors and verify their background information  to qualify them for taking up various works for Al Maha Petroleum Products Marketing Co. S.A.O.G.

The methods of evaluation and due diligence are purely internal and will not be disclosed, in case the contractor is not qualified.

Please verify that all documents and information provided are updated and correct at the time of submission. 

Notes:

1. Any certificate from third party accreditation like ISO 9000 etc.

2. Letter of appreciation from any reputed Client, Consultant or Government Organization
3. Accident / Incident reports

4. Training certificates of staff for HSE

5. List of projects completed in similar industry like ours, giving details of project’s
value and type of work

6. VAT Certificate
